After setting your appointment, you will be directed to the intake forms on this website. Please fill them out and fax or email them to the office two days before your first appointment. Note that there are two versions of the main intake form. The large version can be completed entirely electronically and returned by email.
Your first visit starts with a comprehensive intake. We will go over your medical history, current concerns and wellness goals. Next, the doctor will perform a physical exam to assess your overall health, screen for potential problems and evaluate your chief concerns. You will leave the office with the first steps of a customized plan to help you move toward optimal health. The first visit usually takes 90 minutes. Follow-up visits usually last 45 minutes.
