Signing up for your first appointment
When you make your first appointment, you’ll be prompted to complete and sign the clinic’s electronic paperwork. Please take some time with the extensive new-patient intake form. It covers a lot of ground, and includes areas to submit your insurance information (if we’re billing for you), recent labs or other documents.
If you don’t have copies of your recent labs, please fill out the records release form and send it to the physician who ordered them. That office will send the results here directly.
All preliminary forms are available through the online booking portal. Please give yourself at least 30 minutes to complete the full intake form. Feel free to call the office if you have questions. Use this form to check your insurance coverage, and this one to get your medical records sent directly to Dr. Izakson.
We will meet using a secure, HIPAA-compliant video portal called doxy.me. It’s just like meeting in person, but you can do it from the comfort of your own home! All you need is a good internet connection, decent lighting and privacy. No downloads are required.
The link is always in your reminder email. Just log in at our appointment time and I’ll be right with you.
What to expect at your first visit
Your first visit starts with a comprehensive intake. We will go over your medical history, current concerns and wellness goals. Please have with you all medications and supplements you’re currently taking.
You will leave the appointment with the first steps of a customized plan to help you move toward optimal health.
The first visit usually lasts 90 minutes to 2 hours. Follow-up visits usually last 45 minutes.
Billing and other expenses
Full payment is collected at time of service except where we are billing insurance. Dr. Izakson works with Efficient Medical Billing Services for all insurance issues. To check your insurance benefits, use this form.
Most supplements are available through our online medicinary and are delivered directly to your door. Some items are available for local contactless pickup at the clinic in Portland or Anchorage. Custom formulas are shipped via USPS for patients outside these areas.
The supplements Dr. Izakson prescribes are generally not covered by insurance, although some people have success getting reimbursed through health savings accounts. Many patients spend about $6 daily for their herbs and supplements.
In fairness to our patients and our staff, we have and enforce a 24-hour cancellation policy. If for any reason you need to cancel or reschedule, please do so through our online scheduler (most reliable) or by calling or emailing the office. If you miss your appointment or cancel with less than 24 hours notice, you will be charged the full price of your visit. These fees are not covered by insurance.
