• Moda (Endeavor Select + Pioneer networks)
• Multiplan (Primary, Complementary, Auto and Workers’ Compensation networks)
• Premera Blue Cross Blue Shield (Global, Heritage, Legacy and Yukon networks)

With patients for whom Dr. Izakson is an in-network provider, we will automatically bill your insurance company and let you know if additional payments are required. All insurance billing is done through Efficient Medical Billing Services. If you have questions about your bill, please contact them directly.

If you are insured through a company Dr. Izakson is not in network with, we require payment at time of service. In those cases we offer both a cash discount and all the paperwork you need to submit for insurance reimbursement.
